Medical Records

For a copy of your medical records:
1. Please click the link below to submit a medical records request; DO NOT submit your request in person.
2. There will be a $6.50 charge for your medical records.
3. If you need imaging (e.g. MRI, X-ray, CT scan) in addition to medical records, there is a $15 charge for digital images. Please use the same link below. If you would like your images on a CD instead, please email recordrelease@concordortho.com.
4. You should receive your record within 5-7 business days from payment for your request.
