To our referring physicians, health care providers, and managed care companies, here are some tools to help us serve you better.

Concord Orthopaedics offers an "Appointment by Fax" form for our referring physicians. Simply complete this form, fax it to our office, and we will contact your patient within 1-2 business days to schedule an appointment. If requested, we will notify you that we have contacted the patient and scheduled an appointment.

To print out a copy of our "Appointment by Fax" form, click here. Our fax numbers (as listed on the form) are:

  • Concord, 603.228.7268
  • Derry, 603.426.5223
  • New London, 603.526.5402
  • Windham, 603.870.0072

To make an appointment by phone, call 603.224.3368 or 800.660.2672 and speak to one of our patient care coordinators. We appreciate the opportunity to be of assistance in the care of your patients!

In our on-going effort to assure that we are meeting the needs of our referring physicians, health care providers, and managed care companies, we offer “On-The-Road” visits to your facility. These visits give us the opportunity to hear from you about how we are doing and how we can improve.

“On-The-Road” visits include a physician and a member of our leadership team.

If you would like to request an “On-The-Road” visit from Concord Orthopaedics, please contact Matt Johnson, our Marketing Manager, at 603.224-3368, or by e-mail,

If you are dissatisfied with the service or response time provided by Concord Orthopaedics, please initiate a "Service Alert" by calling our main switchboard, 603.224.3368, and letting our operator know this is a "Service Alert." You will be connected with a manager who will address your concerns.

To our Patients, Families and Communities,

Your health, welfare & safety are paramount when you come for an appointment at any of our facilities. We are closely monitoring the guidance of the World Health Organization and the Centers for Disease Control & Prevention regarding the spread of the virus. Our focus is to ensure we meet our patient’s needs while doing our part to keep you, our associates, and our communities safe. Effective immediately, the Acute Injury Clinic (AIC), will remain open Monday-Friday from 9 a.m. - 4 p.m. The AIC will NOT open on Saturdays.

At this time, the impact to our communities has been minimal and our staff is focused on providing the orthopaedic care our patients expect and deserve. If you have an appointment with us, we respectfully ask that you let us know if you have a fever, cough, and/or shortness of breath in advance of your appointment. Also, if you have recently traveled to or been in close contact with anyone who has traveled outside of New Hampshire, especially to New York State, we ask that you avoid visiting our practice for a period of 14 healthy days.

Thank you for your continued trust in COPA. We appreciate your cooperation as we work to support the health of our patients & employees.
