We have provided forms and other helpful information to assist you in preparing for your visit at Concord Orthopaedics.

Request Medical Records/X-Rays
Use this form to request a copy of your medical records or X-rays to be sent to you or another health care provider. Requests for release of medical records from an insurance company or attorney must be in writing and include your signed authorization form.

Prescription Refill Request
Use this form to request a prescription refill for a non-narcotic prescription. Please allow 1 to 2 business days, excluding weekends, for your prescription refill request to be processed. We will notify you when your prescription is available.

Pay Your Bill
Use this form to pay your bill using MasterCard®, Visa®, Discover®, and American Express®. We will send you a receipt of payment within 48 hours.

Update Account Information
Use this form to update your account information such as change of name, change of address or updated insurance information.
