Appointment by Fax
Concord Orthopaedics offers an "Appointment by Fax" form for our referring physicians. Simply complete this form, fax it to our office, and we will contact your patient within 1-2 business days to schedule an appointment. If requested, we will notify you that we have contacted the patient and scheduled an appointment.

Would you like us to visit your practice?
In our on-going effort to assure that we are meeting the needs of our referring physicians, health care providers, and managed care companies, we offer “On-The-Road” visits to your facility. These visits give us the opportunity to hear from you about how we are doing and how we can improve.
“On-The-Road” visits include a physician and a member of our leadership team.

Service Alert Procedure
If you are dissatisfied with the service or response time provided by Concord Orthopaedics, please initiate a "Service Alert" by calling our main switchboard, 1.603.224.3368, and letting our operator know this is a "Service Alert." You will be connected with a manager who will address your concerns.
